The Illinois Health Facilities and Services Review Board voted 8-0 to approve a $231 million replacement hospital for Touchette Regional Hospital in Cahokia Heights. The new 108-bed facility will rise on a 35-acre site at 1928 Sandy Ridge Road. It replaces Touchette's current 65-year-old building on Bond Avenue, where emergency access is frequently delayed by railroad crossings and occasional flooding.
The replacement hospital will include 96 inpatient behavioral health beds and 12 medical/surgical beds. It will also add an emergency department, operating rooms, and upgraded imaging services. The behavioral health beds break down into 24 for adolescents ages 12 to 17, 24 for geriatric patients, and 48 for adults. Hospital officials say the upgrade will improve access for patients while helping recruit physicians to one of Illinois' most medically underserved regions.
The approval came despite a recommendation against the project from state review board staff. Staff pointed to an existing surplus of mental illness and medical-surgical beds in the region. They also raised concerns about redundancy with existing services, Touchette's recent financial performance, and construction costs. That same staff report acknowledged the Metro East's ongoing health care access problems, including transportation barriers and a high share of Medicaid and uninsured patients at the current facility.
Funding is heavily weighted toward state support. State grants will cover $222.75 million, about 96% of the total cost, with the remaining $8.25 million financed through a mortgage. Of that state funding, $15 million is set aside specifically for the hospital's adolescent behavioral health wing. Construction is expected to begin in March 2027, with completion targeted for June 30, 2030.

The most telling detail here isn't the project itself, it's that the board's own staff recommended against it. Staff cited a regional bed surplus and flagged Touchette's financial performance as a risk, yet the board still voted 8-0 to approve. For GCs and subs tracking healthcare work in underserved markets, that's a useful data point: need and access can outweigh a surplus finding when a facility serves a genuinely underserved population.
The funding structure reinforces that read. At 96% state-funded with just $8.25 million financed privately, this project carries minimal owner risk exposure compared to a typical mortgage-heavy build. That's the kind of capital position that tends to keep a project moving once it clears approval, since there's little financial incentive on the ownership side to delay or rebid.
With construction set to start in March 2027 and run through mid-2030, subs and suppliers in the Metro East have a defined multi-year window to position themselves ahead of the bid process.
